The ALL4ONE™ Breast Form Fund
The ALL4ONE™ Breast Form Fund is a national program that provides up to $300 toward a breast prothesis every 2 years (24 Months).
If you have had surgery to treat or prevent breast cancer you may qualify. Eligibility is based on your financial situation and your diagnosis.
You may be eligible if you can answer yes to ALL of the following questions:
- I have a household income below 250% of the federal poverty level (FPL), see table.
- I have a doctor order or referral for breast prosthesis I need.
- I have had surgery treat or prevent breast cancer
- I have been fit for my prosthesis by a Certified Mastectomy Fitter

Where do I go to be fit for my prosthesis?
The Cancer Resource Foundation and the All4One Alliance work with many stores and boutiques across the country. The program only provides prosthesis through one of our registered stores/boutiques. To find a registered store near you Click Here.
Where to I get my prosthesis?
The program works with stores and boutiques across the country to provide support for women in need. You can only receive product from a registered store/boutique.
Certified Mastectomy Fitters (CMF) are healthcare professionals who are specially educated and trained to fit you properly for breast prosthesis. The CMF completes rigorous training to ensure that you receive a professional fitting service. The certifying organizations are BOC International (Board of Certification/Accreditation) and ABC (The American Board for Certification in Orthotics, Prosthetics, and Pedorthics) hyperlink.
Why do I need to go to a Certified Mastectomy Fitter?
To ensure consistency and professional fitting, we only work with stores and boutiques that provide a fitting from a Certified Mastectomy Fitter. If you are not sure where you find a Certified Mastectomy Fitter you can check with the certifying organizations above, check with your doctor or call us to find a fitter near you.
How do I know if I am approved for your program?
Once you apply in 1SourceApp you will be notified when your application has been approved. Be sure to include your email address when completing your application so you will receive notifications.
How do I receive the money to pay for my prosthesis?
The program pays the store/boutique directly. Once you have been approved for the program, you will go to the store/boutique to get your fitting. The Certified Mastectomy Fitter (CMF) will help pick out the prosthesis that fits best for you. The CMF will check with your insurance company to see how much they will pay toward your prosthesis. You should have a good idea of your “out of pocket” costs after the CRF communicates with your insurance company.
What if I don’t have health insurance?
If you don’t have health insurance, the Certified Mastectomy Fitter (CMF) will fit you for the most appropriate prosthesis and notify us about the cost. The fund will pay up to $300 every 2 years. You will owe the store/boutique for any outstanding costs over $300. The fund will pay the store/boutique directly.
What if the store does not bill health insurance?
If the store/boutique does not bill health insurance, you may have to submit the paperwork yourself, before we can reimburse the store. It is very important that we reimburse for the applicant’s “out of pocket” costs only. We must assure that our funds are only used to pay for the costs of prosthesis not covered by insurance. You may need to ask the CMF for the information needed to bill your insurance company. We provide reimbursement to the store/boutique only after insurance payments have been completed.
