The ALL4ONE™ Garment Fund
The ALL4ONE™ Garment Fund is a national program that provides up to $100 per year toward post-mastectomy or post-lumpectomy garments.
If you have had surgery to treat or prevent breast cancer you may qualify. Eligibility is based on your financial situation and your diagnosis.
You may be eligible if you can answer yes to ALL of the following questions:
- I have a household (hyperlink to definition of household) income below 250% of the federal poverty level (FPL),
- I have a doctor order or referral for the products I need,
- I have had surgery treat or prevent breast cancer.
How to Apply
All applications are processed online using 1SourceApp™. We do not accept paper applications. Applicants register on /patientassistance only ONCE. Where do I get my garments?
The program works with many stores around the country. You can only receive post-mastectomy or post-lumpectomy garments from one of our registered stores. To find out if there is a registered All4One™ Alliance store near you, Click Here.
How do I know if I am approved for your program?
Once you apply in 1SourceApp you will be notified when your application has been approved. Be sure to include your email address when completing your application so you will receive notifications.
How do I receive the money to pay for my garments?
The program pays the store/boutique directly. Once you have been approved for the program, you will go to the store/boutique to get select your garments. The Certified Mastectomy Fitter (CMF) or store sales person will help pick out the bras or garments best for you. The store/boutique will need to check with your insurance company to see if they will pay anything toward your garments before we can send them a check. The store/boutique should be able to let you know your “out of pocket” costs once they have communicated with your insurance company.
What if I don’t have health insurance?
If you don’t have health insurance you still may qualify. This program will pay up to $100 every year (12 months). You will owe the store/boutique for any outstanding costs over $100. The program will pay the store/boutique directly.
What if the store does not bill health insurance?
If the store/boutique does not bill health insurance, you may have to submit the paperwork yourself, before we can reimburse the store. It is very important that we reimburse for the applicant’s “out of pocket” costs only. We must assure that our funds are only used to pay for the costs of garments not covered by insurance. You may need to ask the CMF for the information needed to bill your insurance company. We provide reimbursement to the store/boutique only after insurance payments have been completed.
