National Genetic Testing Co-Pay Assistance Program

Anyone who completes an application will be placed on a waitlist. When funding becomes available, funds will be awarded on a first-come, first-serve basis.
This program is open to individuals from any of the 50 United States. The program provides up to $520 toward the applicants “out of pocket” costs for genetic testing. Applicants must meet eligibility criteria. Funding is awarded on a first-come, first-served basis. Funding for this program is limited. Applications exceed funding on a monthly basis. As funding allows applications will be approved.
Patients must meet ALL criteria below to qualify.
- Applicant’s household income is below 400% of the federal poverty level (FPL)
- Applicant has health insurance (including federal programs)
- Applicant’s doctor, healthcare provider or genetic counselor determines that applicant meets the medical criteria for the test(s). National guidelines for medical determination:National Comprehensive Cancer Network (NCCN) and United States Preventive Services Task Force (USPSTF). When medical criteria are not available through NCCN or USPSTF, applicant’s healthcare provider will need to provide medical justification for ordering the test(s).
